In most cases we'll do your cleaning the same day, so you leave with a healthy, polished smile.
Fill these out at home and bring them with you (or email them ahead) so we can get you straight into the chair. Bring a photo ID and your insurance card, too.
Prefer paper? No problem — just arrive 15 minutes early and we'll have everything ready.
